Inventory Control Property Disposition Instructions
Before completing this form please consider posting your item on the Surplus Equipment Redistribution and/or Disposal website. This website allows departments to post items they no longer need as well as browse for needed items. The website link is www.kent.edu/ufm.
If you need University Facilities Management staff to pick up your item, you must make prior arrangements with them by entering an online work request at www.kent.edu/ufm/ready-work-request. Please have a copy of the Inventory Control Property Disposition form completed for them when they arrive.
- Enter the name and index number of the department disposing of the property.
- Enter the date you are preparing the disposition form.
- Enter the number from the tag that says 鈥淧roperty of 性福五月天 University鈥 located on the item(s).
- Enter a description of the property to be disposed.
- Enter the manufacturer鈥檚 serial number (if appropriate) of the item(s).
- Indicate a Disposal Code for the item(s) as described on the form.
P indicates Picked up by UFM for Disposal
J indicates Junked
ST indicates Stolen (attach copy of police report)
T indicates Traded in (print company traded in to Comments section)
S indicates Sold (print sale amount in Comments section)
L indicates Lost
D indicates Destroyed
- Provide any additional comments which should include (if applicable) the name of the company the item was traded to, or the sale amount (if sold).
- Print this form and obtain the signature of the department head of the disposing department.
- Obtain the signature and print the name of the department head of the disposing department certifying that due diligence has been observed to delete all sensitive data from the hard drive of any computers leaving the department for any reason, other than by theft. Enter the date of signature.
Make 2 copies of the completed and signed form. Give 1 to UFM when they pick up your property and keep 1 copy for your records. Send the original to Inventory Control, 233 Schwartz Center.
